Apart from battling multiple health challenges, they spend significant time coordinating care across multiple providers, explaining issues interpreting current treatment plans, remembering their medication, and tracking appointments.
Connected Care takes a “patient-centric” approach to care co-ordination and management by leveraging technologies like AI and “connecting” all patient information, and giving power to the patient of “sharing” this information with providers and care givers.Decentralized patient data (and lack of access to it) is severely impacting health and peace of mind for the elderly patients and their care givers. Large providers are trying to address the situation by setting up care co-ordination units but are limited by their organizational boundaries. Care co-ordination units are also limited by human training and bandwidth resulting in burn-out of care professionals.
