About Connected Care
Imagine a day in the life of Medicare/Medicaid and elderly patients. Apart from battling multiple health challenges, they spend significant time coordinating care across multiple providers, explaining issues interpreting current treatment plans, remembering their medication, and tracking appointments.
Connected Care takes a “patient-centric” approach to care co-ordination and management by leveraging technologies like AI and “connecting” all patient information, and giving power to the patient of “sharing” this information with providers and care givers.Decentralized patient data (and lack of access to it) is severely impacting health and peace of mind for the elderly patients and their care givers. Large providers are trying to address the situation by setting up care co-ordination units but are limited by their organizational boundaries. Care co-ordination units are also limited by human training and bandwidth resulting in burn-out of care professionals.
To solve these problems, we believe that patients (and their care givers) must be able to:
- Provide a view of the names and specializations of primary care physicians and the specialists to the elderly and care givers based on location of where they reside
- Find and engage suitable providers efficiently, based on several factors including medical coverage, specialty, proximity, and availability
- Have access to all patient data instantly and ability to view the data in one location including, but not limited to, care plan, medications and schedule, providers notes and prescriptions
- Share information with providers and care givers at the click of button, securely and on need basis
- Manage care (appointments, medications etc.) through a human friendly, intuitive application
AI-driven Connected Care is envisioned and built by professionals that have first-hand experience in dealing with patient care issues. While we leverage AI and technology to connect data and information, empathy towards patients and caregivers is the backbone of the Connected Care application.
